It is possible that the races that are starting in a different system get destroyed before you locate them, so you never get a chance to learn that they were somewhere else.
The one thing you might notice, is that it seems to behave differently depending on the computer the game is run on. However, it always does it the same way on that particular computer. Thus, the results you get from sector generation is not the same as what others get. Reminds me of a visual basic RND code with no Randomize Timer statement.

Why not give us a temporary troubleshooting cheat? Something you can turn off in a future patch. Something that just eliminates all fog of war and lets us see where everyone is. I'd start a new ship for the sole purpose of just making sectors to see how things are laid out each time.

This is not my experience. Instead I get the scenario described by DarkTraveller practically every time:

I played with the settings when making sectors. I did a challenge sector. It still does the above 2 things every time.
In contrast to DarkTraveller, I rarely modified the settings when creating new sectors. I always used 'Home planet only' and most of the time 'small' sectors, recently switching to 'tiny' because my system cannot handle the former well regarding performance. I played lots of challenge sectors, though, which exhibit the same pattern.

There were a few instances when it wasn't certain that all races except the one starting with me were placed in the same system, initially. But this should've always been attributable to some early colonization activity. Note, that the races in such crowded systems seem to colonize less than the lone starters. This may be due to them being occupied with getting fed up with each other, then fighting out their wars, before they consider spreading out.

I used to get a message and an indication about destroyed races in the relations screen. There they were shown as 'eliminated', even if I never met them, at least until another race was eliminated which might then have replaced the previous eliminated race indicator. I am not sure this still applies with current patches of the game.

Usually (or always?) something like this had happened early in games where I only ever met one other race after leaving the system I had started in.

Challenges are a different story. They are meant for everyone to start in the same location each time. I don't mean they will all start in the same system as each other, but wherever they start the first time, the should start there in future playthroughs of that same challenge.

For me i get 9 of 10 games all the races in 1 System. And i have the feeling that between all races (include drox) the relation's going down really to fast. I tried a diplomatic win but i only have the time to "save" 2 races, already the rumor's for 2 races to be friendly to each other eat's up my credit's(the hard gaind money from the last sector) and i have not really the time to gain money. The only way for me at the moment to save more than two races are gain a lot of mony, in the last sector.
